Long Term Home Care

Our personal care aides (including Certified Nurse Aids and State Tested Nursing Aids) are the eyes, ears and faces of our home health care services. They typically see our clients much more frequently than most other staff and often, more frequently than the clients own family. They typically assist with the day to day care of the client and assists with several daily tasks, including activities of daily living and instrumental activities of daily living. They are typically individuals selected and matched to the client due to their natural empathy.

Skilled Nursing

Skilled Nursing provides excellent clinical care to clients in the comfort of their own homes to help them get back to their feet in the shortest possible time and when necessary, they provide skilled nursing care for extended periods of time. They constantly strive to improve the quality of health of the patients/clients by working closely with their physicians.

Medical Social Work

The Medical Social Workers performs comprehensive in-home assessment of clients to determine their needs and mobilize the necessary resources available to fulfill those needs. Their assessments includes needs pertaining to counselling, community resources, safety, family support, security, education and much more

Physical Therapy

Our Physical Therapy services help clients with mobility challenges regain functionality and independence. This is particularly helpful for post-surgical patients and for patients with arthritis, joint pain, degenerative joint disease, chronic pain syndromes, and gait and balance disorders, musculoskeletal and neurological diseases.

Home Health Aide

Home Health Aides and Personal Care Aides are the eyes and ears of home health services. They see the patients with much higher frequency than other disciplines and assist with activities of daily living.

Occupational Therapy

Our Occupational Therapists provide clients and their families , who are experiencing physical and cognitive challenges, with the support necessary to fully participate in their own care and to live life to its fullest by helping them promote health, and prevent—or live better with—injury, illness, or disability.

A thorough individualized client/client’s family evaluation, is usually the initial step, during which the client/family and occupational therapist determine the clients goals,

Periodical on-going evaluations are performed to ensure that the goals are being met and/or make changes to the intervention plan.

Congestive Heart Failure

Congestive heart failure (CHF), also called chronic heart failure, is an ongoing condition in which the heart muscle is weakened and can’t pump as well as it normally does. Our skilled nurses work closely with clients Physicians to monitor this condition and make the adjustments necessary to ensure comfort and avoid frequent hospitalizations

We provide both simple and more complex infusion services with strict adherence with physician directives.

Live-In Home Care

 Companionship is key to a trusted relationship with our caregivers. We not only strive to help you with everyday tasks but want to develop a caring relationship with you. We provide one-on-one attention and care that cannot compare in other settings.
